Appointment triage system

Due to the current unprecedented demand for GP appointments, we are operating a new system for patients to access same day appointments.

This system will ensure that those patients in most need of a same day appointment are prioritised and allocated an appointment with the most appropriate clinician.

Jubilee Medical Centre operates a triage system, and any request for an appointment will be passed to our triaging clinician on the day. Triage is a method of deciding the best course and provider of treatment for the presenting condition. The clinician will make a decision as to the most appropriate person to deal with the problem.

The ideal way to request an appointment is via e-Consultation, this form will ask you questions relevant to your problem. If you cannot complete an e-Consultation, you may come to reception and a receptionist will be able to help you complete an e-Consultation using an iPad, or they may be able to help you via the telephone.

Same day appointments

One of our clinicians will be evaluating the information that you completed on the e-Consultation or that was given to our care navigators.

In order to run this service efficiently, where appropriate, we will be using text messages to contact patients with the details of their appointment. For those without mobile phones, you will be contacted back by a member of staff with the details.

Following day appointments

To ensure safe practice at all times, as safety is our greatest priority, the clinician may suggest that certain conditions may be more suitably dealt with the following day or at a later time. You will be informed of an appropriate appointment as required.

Having listened to all of the feedback form patients over recent months, both positive and negative, we hope this new system will ensure that we are making the most efficient and effective use of our available resources.
