Up-to-Date Details
It's really important to us that we know how to contact you for a whole range of reasons - from simply changing an appointment to notifying you of an important test result. So please keep us informed if you move, or change your phone number. By logging into Patient Access you can alter your contact details online. The system is secure and links straight into the practice's clinical system, so once you have submitted your details your records can be updated simply and easily by the practice, with no need for the patient to phone or call into the practice.
Please note that the Patient Access site is not operated by Jubilee Medical Centre. If you experience any problems using Patient Access please check the status of the site by clicking here BEFORE ringing the practice. The practice can reset your account and issue a new linkage key and ODS code to link our practice to your account but, unfortunately, we cannot provide any other support if you encounter difficulties using Patient Access such as forgotten passwords etc.
