Steps to New Patient Registration

In order to register with us you need to follow the instructions below:

Step 1 
Confirm that you are still within the practice's catchment area.

Each practice has a catchment area within which they can accept new patients.

Please use this tool below to check if you are in the catchment area, simply enter your postcode and press "Check if postcode is in Catchment Area"

Step 2 
Complete a Registration Form and New Patient Health Questionnaire. 

The Registration Form asks you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the NHS locally to trace your medical records and send them to us.

The New Patient Health Questionnaire asks you for some basic, lifestyle and allergy questions, so we can provide the best service possible from the start.

There are two ways to register at the practice, the quickest and easiest way is to register online, this is a new service introduced in May 2022. If you would rather not do this online, you can print off the registration form and new patient health questionnaire below and bring it to the practice, or alternatively you can collect a registration form from reception in the practice.

Without a correctly completed and signed form the practice cannot proceed to register you as a patient. This only applies if you are not completing the form online.

In order for you to register at the practice, we ask that you provide proof of identity and of residence in our practice area. However, if it is not possible to provide proof of these documents, please contact the surgery to discuss.  Click here for details of the documents we can accept.

Step 3 
Being placed on the Practice list. 

If Registering Online

If you have registered online, your application should have been received electronically. One of our admin team will register you as soon as possible and you should receive your welcome letter shortly after. This can take up to two weeks. If there are any problems, we may contact you using the details provided on your registration form.

If Printing From Home

Bring the completed Registration Form, New Patient Health Questionnaire and evidence of identity and address to the surgery. One of our admin team will register you as soon as possible and you should receive your welcome letter shortly after. This can take up to two weeks. If there are any problems, we may contact you using the details provided on your registration form.

Step 4

New Patient Assessment.

If Registering Online

Once you have been registered at the practice. You will receive a welcome letter inviting you to attend the surgery for a New Patient Health Assessment, this is a brief appointment with our Health Care Assistant who will ask you some simple lifestyle questions. When attending your appointment, could you please bring with you your evidence of identity and address to the surgery.

If Printing From Home

Once you have been registered at the practice. You will receive a welcome letter inviting you to attend the surgery for a New Patient Health Assessment, this is a brief appointment with our Health Care Assistant who will ask you some simple lifestyle questions. When attending your appointment, there is no reason for you to bring any evidence of your identity or address to the surgery as you have already done so when you returned your forms to be registered.
